Move away from a negative management mentality.
Stop looking at staff as liabilities you need to reduce, try to engage them positively and constantly move in a forward direction with regard to innovation, training and inclusiveness.
Vet your leadership, i mean from the lowest levels, get inside the heads of the managers to know what they feel, are they capable of empathy or not; the latter is only going to lead to a long road of bad morale and ultimately spiral into the negative management illness mentioned above.

Pay people adequately for the work they do. Replace staff who leave. Stop outsourcing valuable roles of experienced staff or at least provide adequate training to the offshore teams and allow time for them to understand it before placing them in customer facing roles. Offshore consultants do not know how to do their job (through no fault of their own) and even internal teams find it difficult to do their job now as offshore staff always advise you called the wrong area when you haven't as they do not want to take ownership or admit they do not know how to help you.
