Full job description

NDIS Disability Support Worker – Sydney

Hometown Nursing Care is looking for compassionate, reliable and motivated Disability Support Workers to join our growing NDIS team across Sydney.

You will provide person-centred support to NDIS participants, helping them live independently, participate in their community and achieve their individual goals.

Key Responsibilities

  • Provide personal care and assistance with daily living
  • Support participants with community access and social activities
  • Assist with appointments, shopping and everyday tasks
  • Encourage independence, choice and confidence
  • Follow individual NDIS support plans
  • Maintain accurate progress notes and documentation
  • Work respectfully with participants, families and other professionals
  • Follow Hometown Nursing Care policies and NDIS requirements

What We’re Looking For

  • Certificate III/IV in Individual Support, Disability or a related qualification preferred
  • Previous disability/NDIS experience highly regarded
  • NDIS Worker Screening Check
  • First Aid & CPR
  • Working With Children Check where required
  • Australian working rights
  • Current driver licence and reliable vehicle preferred
  • Excellent communication skills
  • Caring, respectful and dependable attitude
