The Team & Role

The Transition Care Program at Bayside Health – Peninsula Care Group provides multidisciplinary services across multiple sites throughout the Mornington Peninsula. Working within a person-centred Allied Health team, this role supports the delivery of safe and effective patient care prescribed by Physiotherapists, Occupational Therapists, Social Workers, Dietitians and Speech Pathologists.

The role involves supporting Allied Health professionals with client treatment programs, assisting with caseload management, monitoring and reporting client progress, and maintaining accurate documentation. You will work collaboratively with the multidisciplinary team to deliver high-quality care, contribute to service improvement initiatives, and support positive outcomes for clients.

Skills & Experience Required

Certificate 4 in Allied Health Assistant
Demonstrated experience as an Allied Health Assitant
Capacity to work independent and autonomously
Ability to plan and manage own workload
Experience in working within subacute/Aged Care Allied Health settings (desirable)
