What You’ll Be Doing.

You’ll deliver high-quality Support Coordination services, including:

  • Supporting participants to understand and implement their NDIS plans in line with the National Disability Insurance Scheme.
  • Coordinating a range of supports to help participants achieve their goals, including engaging providers, allied health and community services.
  • Working with participants to build capacity, while also addressing more complex barriers where required.
  • Developing and implementing service plans, particularly for participants with higher or more complex needs.
  • Managing and responding to risks, incidents, and crisis situations in line with the NDIS Practice Standards.
  • Completing participant onboarding, including service agreements, assessments and required documentation.
  • Building strong, professional relationships with participants, families and key stakeholders.
  • Liaising with stakeholders and promoting services to support participant outcomes.
  • Monitoring feedback, identifying improvements, and contributing to continuous quality improvement.

What You Need

To hit the ground running, you’ll need:

  • Tertiary qualification or diploma, or equivalent substantial experience and study sufficient to demonstrate competence in performing the duties of this position.
  • Experience in plan management and case management.
  • Specialist Coordinators require experience in supporting participants with complex disabilities with high support requirements.
  • Understanding of NDIS Practice Standards & Restrictive Practice Rules.
  • NDIS Worker Screening.
  • First Aid + Infection Control.
  • NDIS Orientation + Induction Modules.
  • Operate as an independent contractor (ABN required), including responsibility for your own tax and superannuation.
  • Relevant insurances, including Professional Indemnity and Public Liability.
