LifeChoices SupportMake a Difference Every DayWe are seeking reliable, compassionate and experienced Support Workers to join our team supporting clients across NDIS, Supported Independent Living (SIL), disability and aged care.If you are passionate about person-centred support and helping people live with dignity, independence and choice, we would love to hear from you.About the RoleYou will provide high-quality support tailored to each client's individual needs, goals and care plans. Duties may include:

• Personal care and assistance with daily living.
• Medication support and administration in accordance with approved medication plans, training and organisational procedures.
• Community access, appointments and social participation.
• Meal preparation, domestic assistance and daily routines.
• SIL and overnight support, where required.
• Mobility and manual handling support.
• Implementing support, care and behaviour support plans.
• Completing progress notes, medication records and incident reports.
• Promoting client choice, independence, dignity and safety.

Essential Requirements

• Experience in NDIS, disability, SIL, aged care, community services or a similar role.
• Relevant Certificate III/IV in Individual Support, Disability, Ageing Support, Community Services or equivalent, where applicable.
• Current medication administration/medication competency or endorsement, or willingness to obtain required training.
• Current First Aid & CPR.
• Current Working with Vulnerable People Registration (WWVP), NDIS endorsed or willing to obtain the endorsement.
• NDIS Worker Screening Check, where applicable.
• National Police Check, where required.
• Current driver's licence and reliable, appropriately insured vehicle, where required.
• Ability to successfully complete required screening, reference checks and mandatory training.

Desirable

• NDIS/SIL experience.
• Aged care or dementia experience.
• Complex support experience.
• Positive Behaviour Support experience.
• Manual handling and medication experience.
• Experience with client documentation and care records.

Pay: $35.00 – $72.00 per hour
