Your new role:
In this role, you'll be a vital connector, advocate and health support professional for Aboriginal and Torres Strait Islander people living with chronic disease.
You will work collaboratively across hospital and community services to strengthen culturally safe care and empower individuals and families in their health journey.
Coordinate and support chronic disease care pathways, linking individuals and families with the right health and social services.
Deliver culturally appropriate clinical support, health promotion and chronic disease education to improve long-term health outcomes.
Build strong relationships with community, stakeholders and service providers to identify priority needs and drive meaningful action.
Collect and report activity data, contribute to planning, and participate in key meetings to support continuous improvement in Aboriginal and Torres Strait Islander health.
This is an identified vacancy with a genuine occupational requirement for the position to be filled by an Aboriginal and/or Torres Strait Islander person. Demonstrated clinical knowledge in chronic disease care (e.g., diabetes, respiratory, cardiac) and a commitment to ongoing learning.
Ability to work independently, manage your workload effectively and contribute to continuous improvement within the service.
