Full job description

Serene Golden Wellness Solutions is a growing provider of Supported Residential Services (SRS) and NDIS supports. We are looking for a motivated and compassionate Community Services & Resident Coordinator to join our team and grow with our organisation.

This is an exciting opportunity for someone who enjoys working with people, building relationships, solving problems and making a genuine difference in the lives of others. We are looking for someone who is seeking a long-term career with opportunities for professional growth and future leadership.

About the Role

You will work closely with residents, families, hospitals, allied health professionals and community partners to coordinate supports and improve participant outcomes.

Your responsibilities may include:

  • Coordinating resident and participant supports
  • Liaising with families, support coordinators and healthcare professionals
  • Assisting with admissions and service planning
  • Maintaining accurate documentation and case notes
  • Building positive relationships with community organisations and referral partners
  • Supporting continuous improvement and service development
  • Working closely with management to help grow our services

About You

We are looking for someone who has:

  • A background in Community Services, Social Work, Psychology, Mental Health, Human Services or a related field
  • Excellent communication and interpersonal skills
  • Strong organisational and problem-solving abilities
  • A genuine passion for supporting vulnerable people
  • Confidence working independently as well as within a team
  • A positive attitude and willingness to learn
  • The desire to build a long-term career within a growing organisation

Experience in SRS, NDIS, disability, aged care or mental health is highly regarded but not essential.
