Join Our Team as a Social Worker - Mental Health & Community Support - Make a Meaningful Impact!
OUR COMPANY
HelloDR Blackstone is a busy, welcoming medical centre at 1/12 Hill Street, Blackstone, in the heart of Ipswich, open 7 days a week to serve patients across Booval, Blackstone, Bundamba and the wider Ipswich region. Under one roof we bring together general practice, allied health, onsite Mater Pathology and Designated Aviation Medical Examiner (DAME) services with dental, radiology and cosmetic services coming soon. We're a growing, community-focused, bulk-billing-friendly practice.
Our expert team specialises in managing acute, non-life-threatening injuries and illnesses that would typically require a visit to the Emergency Department.
The HelloDR Blackstone operates during the following hours:
Monday to Friday: 8:00 AM to 7:00 PM
Saturday & Sunday: 8:00 AM - 4:00 PM
The Role
Make a meaningful difference by supporting patients facing mental health, social, and community challenges. As our Social Worker, you'll provide compassionate psychosocial support, helping patients navigate life's challenges while connecting them with the services and resources they need.
Working closely with our GPs and allied health team, you'll contribute to holistic, patient-centred care through Medicare Mental Health Treatment Plans and other referral pathways, ensuring each patient receives the right support to improve their overall wellbeing.
Key Responsibilities
- Provide short-term counselling and psychosocial support to patients referred by our GPs
- Support patients navigating housing, financial, family or community pressures alongside health concerns
- Coordinate referrals to external mental health and community services
- Maintain accurate, sensitive clinical documentation
- Advocate for patient wellbeing
About You
- Bachelor's degree in Social Work (or equivalent) with AASW eligibility
- Accredited Mental Health Social Worker status highly regarded
- Experience providing trauma-informed, person-centred care
- A steady, grounded presence — this role sees people on hard days
- Compassionate and patient-focused
