Key Responsibilities include:

· evaluating client needs and designing, coordinating and delivering tailored support, education and training programs

· conducting interviews to determine the scope and seriousness of client challenges

· tracking client outcomes and preparing regular progress updates

· directing clients to suitable organisations for specialised assistance

· identifying community needs and reviewing the availability of services for health, housing, employment, education, and welfare

· collaborating with local groups, service providers, government departments and businesses to raise awareness and address community concerns

· supporting families and delivering care for children and people with disabilities in group homes, care facilities and service centres

· helping young individuals address social, emotional, and financial difficulties

· drafting funding proposals, resource applications and formal reports for government and other stakeholders

Requirements:

  • A relevant diploma or higher qualifications with specialisation in community services.
  • Experiences within a community/home care/residential care/disability care/youth work setting desired.
  • Outstanding interpersonal and communication skills.
  • Ability to commute to client homes and different residential facilities.
  • Valid driver's license and access to dependable transportation.
  • Police Check.
  • Up to date with Flu & COVID-19 Vaccinations
