Full job description

Anchored Care is seeking a motivated and compassionate Community Worker to join our team in delivering high-quality community-based support services across disability and community care programs. This is a full-time position based in Australia.

Key Responsibilities

· Conduct initial client consultations to understand individual needs, challenges, and support requirements

· Develop, implement, and review tailored support plans and community participation programs

· Provide ongoing monitoring of client progress and adjust support strategies as required

· Support clients in accessing relevant community services including health, housing, employment, education, and welfare supports

· Liaise with government agencies, community organisations, and service providers to coordinate appropriate assistance

· Promote awareness of available community resources and facilitate client engagement with support networks

· Assist individuals and families, including people with disabilities, young people, and vulnerable community members, to improve wellbeing and independence

· Provide guidance and practical support to help clients manage social, emotional, and daily living challenges

· Prepare case notes, reports, and documentation for internal use and external stakeholders as required

· Contribute to service development by identifying gaps in community needs and available supports

· Ensure compliance with organisational policies, care standards, and funding requirements

Skills and Experience Required

· Previous experience in community services, disability support, social work, or a related field

· Strong ability to assess client needs and develop appropriate support strategies

· Excellent communication, interpersonal, and case management skills

· Ability to work with diverse client groups including vulnerable individuals and families

· Strong organisational skills with attention to documentation and reporting

· Ability to liaise effectively with external agencies and multidisciplinary teams

· Empathy, patience, and commitment to client-centred care

Qualifications

· Diploma or higher level in Community Services, Individual Support, Disability, Youth Work, or related field (preferred)

· Relevant experience may be considered in lieu of formal qualifications

Employment Conditions

· Full-time ongoing position

· May include flexible shifts, including evenings and weekends depending on client need
