Full job description
About the Role
We are seeking an experienced and dedicated Support Coordinator to join our team. The successful candidate will play a key role in empowering NDIS participants to understand, implement, and get the most out of their plans connecting them with the right supports, building their capacity for independence, and ensuring their goals remain at the centre of everything we do.
Key Responsibilities
- Work directly with NDIS participants to understand, interpret, and implement their NDIS plans
- Connect participants with appropriate mainstream, community, and funded supports
- Monitor and manage participant budgets, ensuring funds are used effectively and in line with plan goals
- Navigate and manage NDIS systems and portals (including PACE and the NDIS provider/participant portals) to process claims, review plan details, and manage participant records
- Liaise with the NDIA, Local Area Coordinators (LACs), and other stakeholders on behalf of participants
- Support participants through plan reviews, reassessments, and any changes to funding or circumstances
- Build strong, trust-based relationships with participants, families, and carers from a wide range of backgrounds
- Identify and respond to participants' individual needs, including complex or high-support requirements
- Maintain accurate, up-to-date case notes and documentation in line with organisational and NDIS Practice Standards
- Support participants in crisis or urgent situations, escalating appropriately where required
- Ensure all work is compliant with the NDIS Code of Conduct and relevant legislation
About You
- Minimum 5 years' experience working as a Support Coordinator or in a closely related NDIS/disability support role
- Strong working knowledge of PACE and the transition from the legacy myplace portal, and confidence navigating NDIS provider and participant portals
- Solid understanding of NDIS plan budgeting, including core, capacity building, and capital supports, and how to manage funds efficiently across a plan period
- Clear understanding of NDIS access and eligibility criteria, plan types, and support categories
- Demonstrated experience working with participants presenting with a diverse range of needs and challenges, including psychosocial disability, intellectual disability, physical disability, complex behavioural needs, and dual diagnoses
- Excellent communication, negotiation, and relationship-building skills
- Highly organised, with strong time-management skills and the ability to manage a caseload independently
- Sound understanding of the NDIS Practice Standards and Code of Conduct
- Current NDIS Worker Screening Check (or willingness to obtain)
- Current Working with Children Check (if applicable)
- Relevant tertiary qualification in social work, disability, community services, or a related field is highly regarded
- Valid driver's licence and access to a reliable vehicle
Hours
Full-time position 5 days per week, 8 hours per day (40 hours per week).
Pay: $43.00 – $45.00 per hour
