Access Lifecare is a registered NDIS service provider supporting participants across the ACT and Queensland. We are committed to delivering person-centred disability services that empower individuals to achieve independence, community participation, and improved quality of life.

We are currently seeking a compassionate and motivated Disabilities Services Officer to join our growing team.

About the Role

The successful candidate will work closely with people living with intellectual, physical, social, and emotional disabilities by coordinating supports, facilitating community access, and assisting participants in achieving their NDIS goals.

Key Responsibilities

  • Assess participant needs and assist in developing support plans
  • Coordinate disability and community support services in line with NDIS plans
  • Monitor participant progress and maintain accurate case documentation
  • Refer participants to appropriate community, welfare, housing, health, and support services
  • Liaise with families, carers, service providers, government bodies, and community organisations
  • Assist participants in accessing education, employment, social, and recreational opportunities
  • Support participants in exercising choice and control regarding their supports
  • Promote participant independence, inclusion, and well-being
  • Assist with funding reviews, service coordination, and support arrangements
  • Ensure compliance with organisational policies, NDIS Practice Standards, and legislative requirements

Essential Criteria

  • Relevant qualification in Community Services, Disability, Social Science, Welfare, or related field
  • Minimum one year of relevant experience in disability support or community services
  • Knowledge of the NDIS framework and disability support sector
  • Strong communication, interpersonal, and organisational skills
  • Ability to work independently and collaboratively within a team
  • Current Driver's Licence and reliable vehicle
  • NDIS Worker Screening Check
  • National Police Clearance
  • Working with Vulnerable People Registration
  • Full working rights in Australia
