NDIS Support Coordinator (Full-Time)
- Location: Penrith NSW & 2 days WFH following probation
- Work Type: Full-Time
- Sector: Disability Support / NDIS
- Pay: 65K to 80K + Super depending on experience
Are you passionate about empowering NDIS participants to navigate the system, build capacity, and achieve their goals? Do you thrive in a collaborative, close-knit environment where hard work is balanced with a good laugh?
At Assist Disability, we do things a little differently. We believe that the best outcomes happen when professionals collaborate, innovate, and support one another. We are seeking a dedicated, full-time Support Coordinator to join our small, highly supportive multidisciplinary team in Western Sydney.
- We set you up for success, not burnout. While this is typically not your 9-to-5 role, you will start with a lower billable target and a warm pipeline of initial participant referrals, giving you the space to comfortably build your caseload over your first few months.
- Set KPI: 25 billable hours per week.
- A small team that works hard, shares knowledge, research, collaborates, and is always ready for a laugh.

As a Support Coordinator, you will work holistically with participants across various age groups (children, young adults, and adults) to connect them with mainstream, community, and provider networks.
Responsibilities:
- Support participants to understand, implement, and maximize their NDIS plans.
- Budget planning and management to ensure participants get the most out of their plans for the life of the plan.
- Travel across Western Sydney as required to conduct in person visits with participants and their support networks in their home.
- Foster capacity building, independence, and resilience within vulnerable participants.
- Coordinate, research and follow up multi-agency service delivery, working closely with internal PBS practitioners and external providers.
- Conduct research and phone calls to providers to present participants with multiple options for providers.
- Write comprehensive evidence-based progress reports and advocate effectively for participants during NDIS plan reviews.
- Manage your caseload efficiently to meet the 25 minimum billable hours per week milestone once fully ramped up.
- Maintain accurate and up-to-date records of client interactions and progress.
Requirements:
- Current valid Australian driver’s license and have access to a reliable, registered vehicle to perform community-based visits.
- Strong knowledge around disability, the NDIS ecosystem, and supporting vulnerable people. This role requires empathy, compassion, and understanding for people from all walks of life.
- Excellent written and verbal communication skills for report writing and negotiation to advocate effectively for your participants' funding and needs, communication with providers and effectively conduct research.
- Computer literacy for managing spreadsheets, NDIS pages, internal CRM software, report writing and research.
- You love being part of a small team, are willing to share your own knowledge, research and appreciate a workplace that values both hard work and a positive, lighthearted atmosphere.
- Experience in a support or care role is advantageous, though entry-level candidates are encouraged to apply.
- Current NDIS Worker Screening Check, Employee/ paid Working with Children Check (WWCC), full work rights and driver's license.
