Summary:

The Support Coordinator plays a vital role in the social care sector, providing essential support to individuals and families in Geelong, Victoria. This position is designed for entry-level candidates who are passionate about making a difference in the community. The Support Coordinator will assist clients in navigating services and resources, ensuring they receive the necessary support to enhance their quality of life.

Responsibilities:

  • Assist clients in identifying their needs and goals to develop personalised support plans.
  • Coordinate and facilitate access to services and resources for clients.
  • Maintain accurate and up-to-date records of client interactions and progress.
