About the role

Assist NDIS participants in coordinating and implementing support services based on their NDIS plan, using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery. This is a permanent full time position located from South Morang or Dandenong. You will have the flexibility to work from home but will be required to meet with clients across the region as required.

Key responsibilities

  • Strengthen and enhance our client's ability to coordinate supports and participate in the community
  • Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
  • Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
  • Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency

Skills & experience

  • A professional and positive outlook about supporting people with disabilities and commitment to supporting and promoting the individual needs, skills, abilities and personal goals of people with disabilities and mental health concerns.
  • Commitment to the values of dignity, respect, inclusion, empowerment, confidentiality and strength-based practice.
  • Demonstrated experience in managing competing priorities and to deliver outcomes within agreed timeframes and quality standards.
  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the National Disability Insurance Scheme
  • Unrestricted drivers licence and willingness to travel as required
  • Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
  • Established time management, problem solving and conflict management skills

Successful candidates will be required to clear probity checks including National Criminal History Record Check (paid for by us), NDIS Workers Screening Check and a VIC Working with Vulnerable People Check (paid for and obtained by you).
