We have a position available for an enthusiastic and experienced male Support Worker in the northern suburbs of Perth.

The Role

Using a person-centred and active support approach, a Support Worker will work as part of a team to provide support services to people with disabilities. The Support Worker will work with a range of plans for each client, such as an individual Support Plan, Health Care Plan, Medication Plan, Meal Plan, and Manual Handling Plan, and will work within the rhythm of service for each house.

Key Responsibilities:
  • Maintain and monitor participants' support plans
  • Ensure a participant-focused and person-centred approach is maintained when engaging with a client
  • Provide direct support and assistance to individuals with disabilities in daily activities, including personal care, meal preparation, and household tasks
  • Assist with mobility and transportation needs, ensuring safety and comfort
  • Support participants in achieving their personal goals and enhancing their quality of life
  • Monitor and report on participant progress, behavior, and any changes in health or well-being
  • Maintain accurate records and documentation of care provided
  • Maintain professional and effective interactions with team and stakeholders
  • Fulfill compliance and training requirements
  • Must be prepared to work over a 24/7 roster, including mornings, evenings, weekends and sleepovers
  • Must be available to travel around Perth metropolitan areas

Skills and Essential Requirements
  • Minimum Certificate III in Disability, Individual Support or a related field
  • NDIS Worker Screening Check and WWCC
  • Current First Aid & CPR Certificate (HLTAID003 & HLTAID001)
  • Current Manual Handling Certificate
  • Current Assist with Medication Certificate
  • Driver's License
  • Working Rights in Australia
  • Must have a car and car insurance
  • Must be fluent in English
