Full job description

FULL-TIME Disability Support Workers — Real People. Real Heart. Real Impact.

Multiple locations | SCHADS + penalties

We’re looking for people who show up, care deeply, and do the right thing — even when no one is watching.

Concept Care is growing — and we’re building a team of Support Workers who are dependable, genuine, and committed to making a real difference for people living with disability.

Yes — we work with complex needs.
But what matters most to us is your attitude:

  • you’re reliable and take responsibility
  • you use common sense
  • you’re calm under pressure
  • you treat people with dignity and respect
  • you’re willing to learn, ask questions and improve

If that sounds like you, we want to meet you.

About the role – FULL-TIME EMPLOYMENT

You’ll support people in their homes and communities — helping them live safely, independently and confidently.

Your work may include:

  • everyday living support, personal care and medication prompts
  • assisting with clinical care such as PEG feeding, bowel care, stoma care and catheter support
  • supporting transport to appointments, shops and social activities
  • using positive behaviour support strategies when challenges arise
  • encouraging independence, skills building and community participation

No two days are the same — and your presence genuinely matters.

Who will thrive in this role?

Experience with complex behaviours, mental health, ABI, Autism or high-support needs is highly valued — but the right mindset is just as important.

We’re interested in people who are dependable, compassionate and professional.

You’ll be a great fit if you:

  • have strong communication and de-escalation skills
  • stay steady and thoughtful when things get tricky
  • understand boundaries, privacy and professionalism
  • follow processes and documentation accurately
  • show respect to clients, families and colleagues
  • are open to feedback, training and growth

This role is NOT suited to people looking for:

  • Short-term or ad-hoc casual shifts
  • A second job to ‘fill gaps’
  • Inconsistent availability or frequent roster changes

Certificate IV in Disability/Aged Care/Community Services is preferred, but not essential if you bring the right attitude and willingness to learn.

FULL-TIME BENEFITS (WHY THIS ROLE IS DIFFERENT)

✔ Guaranteed, consistent FULL-TIME hours (38 Hours per week)
✔ Structured rosters — no scrambling for shifts
✔ Job security with a growing NDIS provider
✔ Ongoing training, mentoring & skill development
✔ Clear pathways into senior support, specialist or coordination roles
✔ Supportive leadership — you’re not just a number here

Ready to join a specialist, supportive team?

Apply now — and tell us why you’re someone people can depend on.

We’re excited to meet you.

Pay: $32.88 – $47.16 per hour
