Full job description

Mediheal Disability Services is a trusted NDIS services provider, and we pride ourselves on engaging with compassionate skilled people who are competent in disabilities care. We’re now seeking dedicated, caring, and experienced Aged or Disabled Carers to join our team.

About the role:

In this role, you will provide essential and personalised support to clients who are living with disabilities, or high needs individuals such as people who require ventilator support, seizure management, or mobility assistance. The support we provide enables the client to improve the quality of their lives and meet their individual needs by providing person-centred care.

Responsibilities will include:

  • Read client care plans and deliver a high standard of support
  • Accompany and support clients’ daily living needs including tasks such as meal preparation, medication, and other domestic tasks.
  • Provide personal care to clients including mobility assistance, personal hygiene, and dressing tasks.
  • Organise activities for clients to enhance their social engagement and community involvement.
  • Carry out delegated health care tasks as per care plan.
  • Maintain accurate progress notes and incident reports.
  • Work collaboratively with clinical staff, allied health professionals, and families.
  • Promote independence, dignity, and client choice always.
  • Maintain a proactive approach to Occupational Safety and Health (OSH).
  • Maintain privacy, dignity, and confidentiality of all aspects of client care.

You should be able to demonstrate the following qualifications to be considered:

  • Relevant Certificate III in a field such as Individual Support, Home and Community, Disability Care or related field or at least one year of work experience in an accredited residential care or disability care program setting.
