About us:
Located on the edge of Toowoomba, Toowoomba North State School is a supportive and inclusive learning community guided by our vision, 'Strong and smart - Big in heart.' Toowoomba North State School has a rich history dating back to 1869, when it opened as North National School or Mort Estate State School. The schools amalgamated in 1938 to form Toowoomba North State School.
Harlaxton State School is a small, welcoming school in northern Toowoomba with a strong history and valued partnerships across the local community. As a Positive Behaviour for Learning (PBL) school, we are committed to inspiring students to succeed, fostering high expectations and supporting the development of the whole child. Our approach focuses on meeting students' physical, cultural, social, emotional and academic needs, guided by our values of Respect, Relationships and Resilience and a strong commitment to student and staff wellbeing, quality teaching and positive learning outcomes. Your opportunity:
As the Family Support Coordinator, you will work with classroom teachers, school support staff, Early Childhood Coordinators (where available) to establish safe, welcoming learning environments.
Coordinators will assist the school team to support children and families successfully transition into school from early childhood services, facilitate access to learning or engagement support to help establish positive patterns of school attendance, behaviour and engagement in learning and link families to external government and non-government supports and services to reduce barriers to learning for students.
The role encompasses two locations:
Toowoomba North State School (base school) - 0.50FTE
Harlaxton State School - 0.50FTE
Responsibilities:
Build strong partnerships with families, schools, government agencies and community services.
Support students and families with school transition, attendance, behaviour and engagement.
Connect families with appropriate health, therapeutic, developmental and specialist services, including NDIS supports.
Work with teachers and support staff to identify student needs and appropriate interventions.
Support vulnerable children and families and strengthen family capacity and wellbeing.
Develop local networks and coordinate multi-agency support.
Use data to monitor the effectiveness of support strategies and improve student outcomes.
Maintain accurate case and activity records in accordance with legislation and departmental requirements.
