Facilities Coordinator jobs in Tasmania

NDIS Support Coordinators work closely with participants to ensure they maximize the benefits of their NDIS plan.

Key responsibilities include:

• Understanding participant needs: Assessing individual goals, preferences, and challenges to tailor support strategies
• Connecting with services: Linking participants to disability service providers, mainstream services, community resources, and housing or employment supports
• Capacity building: Coaching participants to develop skills and confidence to manage their own supports over time
• Plan implementation: Assisting participants to put their NDIS plan into action, including scheduling services, managing budgets, and ensuring timely delivery of supports
• Problem-solving and advocacy: Addressing barriers, resolving crises, and advocating for participants’ rights and needs
• Reporting and monitoring: Providing progress reports to the NDIA to ensure plans meet participant goals and adjusting supports as circumstances change

Pay: $49.16 – $50.00 per hour

Expected hours: 32.0 per week
