SUPPORT COORDINATOR

FULL TIME ROLE

About the role

MYPAL HOME CARE is seeking a passionate and experienced Support Coordinator to join our dedicated team in Melbourne, VIC. As a Support Coordinator, you will play a vital role in empowering and supporting individuals with disabilities to live independent and fulfilling lives. This is a full-time position that offers a competitive salary and excellent benefits.

What you'll be doing

  • Coordinating and overseeing the delivery of individualized support services to clients, ensuring their needs are met
  • Assisting clients in developing and implementing their goals and support plans
  • Liaising with a range of service providers and stakeholders to ensure seamless service delivery
  • Monitoring and reviewing client progress, and making adjustments to support plans as needed
  • Advocating for clients and their families to ensure they have access to the appropriate services and resources
  • Maintaining accurate and up-to-date client records and documentation
  • Participating in team meetings and contributing to the continuous improvement of service delivery

What we're looking for

  • Diploma in Disability or Community Services related (desired but not necessary)
  • Minimum 1 year experience in a support coordination or case management role, preferably in the aged care or disability sectors
  • Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with clients, families, and stakeholders
  • Strong organizational and problem-solving skills, with the ability to priorities and manage multiple tasks
  • Demonstrated understanding of the NDIS and aged care systems, and the ability to navigate and coordinate services within these frameworks
  • Empathetic, person-centered approach with a genuine commitment to making a positive difference in the lives of clients
  • Current driver's licence and access to a reliable vehicle
