About the role

Careconnect Support Service is seeking casual (becoming part-time permanent if desired) Support Workers to join our team. In this vital role, you will be responsible for providing compassionate and high-quality care to two of our clients, a lady living with a physical disability.

What you'll be doing

· Monitoring and documenting residents' health and progress, and reporting any concerns to our multidisciplinary team

· Organising and facilitating social, recreational and therapeutic activities to enhance the family's wellbeing

· Using the family vehicle to facilitate community access and appointments

· Communicating effectively with our clients, their extended family and the wider care team

· Adhering to all relevant policies, procedures and regulatory requirements

What we're looking for

· Certificate III or IV in Individual Support or equivalent experience

· Proven experience in a Supported Independent Living or similar setting, ideally within the disability support industry

· Excellent communication and interpersonal skills, with a caring and empathetic approach

· Strong organizational and time management abilities, with the capacity to multitask effectively

· A genuine passion for improving the lives of individuals living with a disability, and a commitment to person-centered care

· A current police clearance and the right to work in Australia
