About the opportunity
Deliver clinical Social Work services as a member of the community based multidisciplinary allied health team, for adults/older persons within the Townsville University Hospital catchment.
The role works collaboratively within the multidisciplinary team, as well as across facilities and services to ensure the provision of a quality service, with a focus on complex health conditions, post-acute and rehabilitation focused interventions, and hospital avoidance and restorative programs.

About the department
The services available from Townsville Community Health Services include:
Health assessment and referral.
Discipline specific interventions: Social Work, Psychology, Physiotherapy, Occupational Therapy, Speech Pathology, Nutrition and Dietetics, Podiatry and Exercise Physiology.
Liaison with hospitals, General Practitioners and other services or agencies.
Integrated models of care and shared client service options or programs. Allied Health clinicians work closely with the Community Nursing team and Older Persons services.
The ideal applicant will be someone who has proven ability and can demonstrate the following:
Demonstrated clinical knowledge and skill in providing clinical social work services with community based clients who present with chronic and aging health conditions and/or rehabilitation focused needs.
Demonstrated ability to effectively communicate and work cooperatively within a multidisciplinary team.
Demonstrated understanding of, and ability to apply person centred practice principles, with an understanding and valuing of individualised goal planning and self-management approaches.
Ability to work flexibly and responsively, in a fast paced and complex health environment, with demonstrated problem solving skills and organisational ability.
Demonstrated commitment to ongoing learning and ability to contribute to team quality activities.
Role requirements
The successful applicant must hold a tertiary degree (or equivalent) qualification in Social Work and eligibility for membership with the Australian Association of Social Workers.
This position requires the incumbent to operate a class C motor vehicle and an appropriate licence endorsement to operate this vehicle is required.
Aged Care Pre-Employment Screening
This is a VPD risk role.

We offer attractive remuneration packages and generous leave entitlements.
Salary range: $39.86 - $58.59 p.h. (HP3)
4 weeks annual leave (pro rata) with 17.5% loading
12.75% employer contribution to superannuation
