The Team & Role

The Acute Rehabilitation Program (ARP) is an interdisciplinary service that provides early rehabilitation in an acute setting, working collaboratively to assist patients in regaining their function and independence and facilitate an early supported discharge home where appropriate. As part of this multidisciplinary team environment, the Social Worker provides quality care through competent clinical skills, effective communication and active collaboration, undertaking comprehensive psychosocial assessments and intervention planning in consultation with patients, their families and carers. This role involves prioritising work across multiple wards, responding to referrals, and participating in coordinated patient care, including family meetings and proactive discharge planning. The position requires autonomous practice alongside strong teamwork, contributing to a positive, professional and collaborative work culture, while also supporting mentoring, clinical consultation and supervision of colleagues and students.

Skills & Experience Required

Bachelor Degree or Qualifying Masters in Social Work & be eligible for AASW membership.
Two or more years post-qualifying professional experience in a hospital or related setting.

Ability to supervise staff and/or students

Excellence skills in comprehensive assessment, discharge planning and counselling

Demonstrated knowledge and application of Social Work theory

Demonstrated skills and good working knowledge of community support services.
