Full job description
Welcome to Lutheran Care!
We are an energetic, forward-thinking, not-for-profit organisation, providing support to people who need it most.
In the Northern Territory, we deliver Financial Wellbeing services across Central Australia and are proud to be leading the new Central Intake Service, a Territory-wide initiative improving access to support for people experiencing, or at risk of, homelessness.
Our work in the NT builds on our strong foundation in South Australia, where we provide services such as foster care, homelessness support, emergency relief, family support, counselling and allied health services. Whoever you are and wherever you come from, we will walk alongside you, respecting all cultural backgrounds, beliefs, sexual orientations, and gender identities.
Our programs aren’t just services, we’re breaking down barriers and creating real opportunities for people to belong and thrive.
Lead with purpose. Build systems that change lives.
About the Role
At Lutheran Care, we believe the way we support people shapes the impact we have in the community. We’re looking for compassionate Pathways Workers who thrive on making meaningful connections and helping individuals and families access safe, respectful, and culturally responsive support.
As a Pathways Worker, you’ll be on the frontline, meeting people where they are, listening to their stories and providing short‑term support through risk assessment, brief interventions and coordinated referrals.
You’ll draw on sector frameworks, structured assessment tools and the referral database to complete clear, consistent and trauma‑informed assessments. Working closely with your Team Leader and partner services (Yilli Rreung Aboriginal Corporation, The Salvation Army), you’ll ensure people are connected quickly, safely and appropriately to the supports that best meet their needs.
Key Responsibilities:
- Be the first point of contact for people seeking support, through phone and in-person services.
- Conduct brief, trauma‑informed intake assessments and support clients to understand their options.
- Provide short‑term support only, including brief interventions and warm referrals to partner services for ongoing support.
- Document all client interactions clearly and within required timeframes.
- Respond to disclosures and complex risk presentations using calm, trauma‑informed communication and de‑escalation techniques
- Work collaboratively with partner organisations to ensure clients receive coordinated, culturally responsive support.
What’s in it for You
When you join Lutheran Care, you’ll enjoy:
- 6 weeks annual leave
- Salary packaging to increase your take-home pay
- Relocation reimbursement package
- You’re Amazing Day – an extra day off, just for you
- Paid parental leave
- Free, confidential counselling support
- Flexible working arrangements for work–life balance
What You’ll Bring
- A genuine commitment to supporting people experiencing homelessness, with a respectful and non-judgmental approach.
- An understanding of how to deliver trauma informed services.
- Confidence in responding to urgent situations calmly and effectively, using trauma-informed practices.
- Ability to engage with clients, colleagues, and service providers clearly and respectfully
- Awareness of the unique strengths and challenges of NT communities, alongside an understanding of the ongoing impacts of colonisation and intergenerational disadvantage for Aboriginal and Torres Strait Islander peoples

We’d love to hear from you. Please submit:

At Lutheran Care, inclusion, compassion, courage and social justice guide everything we do.
We know diversity makes us stronger and helps us better support our communities. If you are Aboriginal and/or Torres Strait Islander, living with disability, part of the LGBTIQA+ community, culturally diverse, or bring lived experience, your voice and experience matter to us, and we encourage you to apply.
