About the Role:

Join the RADAR Team and make a real difference in the lives of older people across the ACT. RADAR is a dynamic multidisciplinary service delivering assessment and clinical management to older adults in the community, with the goal of supporting people to remain safely at home and reducing unnecessary hospital admissions. The team includes Geriatricians, Advanced Trainee Registrars, Registered Nurses, Occupational Therapists and Social Workers, working closely with hospital services, GPs and community providers. RADAR also supports clients recently discharged from Canberra Hospital who require short-term follow-up care.

As a Registered Nurse Level 2 with RADAR, you will work in close partnership with Geriatricians, nursing and allied health colleagues to deliver safe, timely and person-centred care in the community. This role offers the opportunity to contribute to short-term assessment and management of older clients, collaborate across disciplines, and play a key role in keeping people well at home.
