Your new role:
As a Mental Health Clinician (Social Worker), you'll provide high-quality, person-centred care to consumers within the Logan Central Adult Community Continuing Care Team, supporting individuals throughout their recovery journey by:
Delivering comprehensive social work assessments, interventions and discharge planning.
Managing a varied caseload within a collaborative multidisciplinary team.
Building strong relationships with consumers, carers and community partners.
Contributing to quality improvement, research and evidence-based practice initiatives.
Supporting student learning and participating in clinical education activities.
Maintaining accurate clinical documentation and delivering safe, high-quality care.
About you:
You'll be a compassionate and motivated Social Worker who enjoys working collaboratively and is committed to delivering exceptional mental health care, with:
A tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
Experience providing evidence-based social work services within mental health or a related clinical setting.
Strong assessment, communication and interpersonal skills.
The ability to work independently while contributing positively to a multidisciplinary team.
Excellent organisational skills with the ability to prioritise a busy caseload.
A commitment to continuous learning, quality improvement and patient-centred care.
