Are you a compassionate, highly organised Service Coordinator. If you are a Disability Experienced Professional looking to make a meaningful impact in the lives of people with disability? We're currently seeking an experienced Service Coordinator to join our dedicated team, supporting both clients and frontline support staff across a community-based disability setting.

What We’re Looking For:

If you’re someone who thrives in a supportive, values-driven environment and brings a genuine passion for client-centred care — we’d love to hear from you. Your industry strengths in areas such as:

  • On-boarding of new clients and staff
  • Support Plan Review
  • Client check-in and monitoring
  • Support Worker Mentorship
  • Excellent Communication with clients, loved ones, internal team members and the broader stakeholder group
  • Report writing; appropriate documentation; NDIS compliance activities.
  • Monitoring shift notes, incident reporting, appropriate follow up and reporting
  • Team work!

We’re looking for someone who brings compassionate confidence alongside a positive, calm, and solutions-focused outlook. Reliability, attention to detail, and a collaborative approach are essential. Most importantly, you’ll be someone who enjoys mentoring and empowering others — while continuing to learn and grow in your own practice.
