Become our next Registered Nurse with Hospital in The Home Central (HiTH). Provide acute and subacute care to patients in their homes.

What You'll Be Doing

As our Intake Registered Nurse for the Hospital in The Home (HiTH) service, you will be the first point of contact for acute hospital referrals. Your day-to-day responsibilities include triaging referrals with clinical judgment to assess urgency and eligibility for home-based care. You will participate in daily team huddles to discuss capacity, risks, admissions, and discharges, ensuring smooth patient flow. Collaboration with HiTH medical staff, clinical leads, and hospital teams is essential to coordinate care transitions. You will communicate directly with patients and families to explain the HiTH model, gather detailed clinical information, and ensure accurate documentation and follow-up of assessments.

In this role you will be supported by the Nurse Manager, Nurse Unit Manager and broader multidisciplinary team. You will also receive a comprehensive orientation through shadowing experienced staff members, system training, and a clear introduction to HiTH referral and triage processes. You will also have the opportunity of professional development through external training relevant to acute care in the community, triage, and virtual care models, encouragement for ongoing postgraduate education and leadership development and participation in clinical governance activities.

Who You Are

Currently registered as a Registered Nurse with AHPRA

A proactive, flexible and resilient RN with strong clinical reasoning and triage skills.

Have excellent communication and interpersonal abilities enable you to liaise effectively with patients, families, and multiple healthcare stakeholders.

Confident to work autonomously while maintaining a team-focused approach, managing complex care coordination with high-level organisation and time management.

Passionate about delivering exceptional patient care in community settings and are eager to engage in continuous learning.
