About you:

This is a rewarding role offering the opportunity to make a real difference in people’s lives. We are looking for:

· Minimum 1 year experience in community services or social support

· Strong communication and interpersonal skills

· Ability to work independently and as part of a team

· Diploma of Community Services (or equivalent)

· A compassionate, reliable, and proactive attitude

· Ability to build positive relationships with clients and stakeholders

· A commitment to promoting inclusion and independence

Tasks & responsibilities:

· Advocate for clients’ rights, access, and inclusion within the community

· Provide person-centred support tailored to individual needs

· Develop and implement individualised support plans

· Support clients with community engagement and participation

· Promote independence, wellbeing, and life skills

· Refer clients to appropriate specialist services and agencies

· Maintain accurate documentation and progress notes

· Work collaboratively with team members and external providers
