About Flagstone State School:
Welcome to Flagstone State School, which is located 9km from the centre of Jimboomba in the ever-growing Flagstone Estate. I am honoured to be principal of Flagstone State School with over 900 students enrolled from Prep to Year 6. Staff work to ensure all students receive quality education, and that their opportunity for success is maximised within a safe and supportive school learning environment.
About the Role:
As the Family Support Coordinator, you will work with classroom teachers, school support staff, Early Childhood Coordinators (where available) to establish safe, welcoming learning environments.
Coordinators will assist the school team to support children and families successfully transition into school from early childhood services, facilitate access to learning or engagement support to help establish positive patterns of school attendance, behaviour and engagement in learning and link families to external government and non-government supports and services to reduce barriers to learning for students.
Responsibilities Include:
Implement structured and long-term collaboration with a diverse range of external partners to drive positive outcomes for families and children to engage successfully with learning and strengthen relationships with parents and carers to facilitate improvement in student attendance, behaviour and engagement in learning.
Work with classroom teachers, teacher aides and specialist support staff to understand the child's needs and connect families with services, such as health, therapeutic and developmental interventions to support engagement with learning.
Build and sustain positive relationships and effective partnerships with students, parents, school staff, government agencies and community services and provide expert input into decision making processes to ensure optimum outcomes for children and their families.
Bring expertise into school communities about vulnerable children and families and their needs to facilitate engagement in learning and improved learning outcomes.
Identify a diverse range of external partners and assist in the establishment of a formal network for support providers and partners.
Connect families and children with services and supports, in the school and externally, that promote access to a range of services in a coordinated and planned way.
Build local capability within schools to strengthen engagement with students and families, improve the quality of classroom learning adjustments and broker multi-agency support.
Respond to individual student needs to provide student and family support to transition to school, establish good patterns of attendance and behaviour and maintain engagement in learning.
Identify children and families needing assistance to access specialist interventions early and facilitate strong connections with school and linkages to other services, including supporting engagement with the NDIS.
Build family capacity to support their child's learning, development and wellbeing, ongoing engagement in school, and assist state schools to respond to the increasingly complex needs of students and families.
Contribute to a whole of community Family Support Coordinator network to ensure key contact relationships are maintained.
Effectively use data to identify, implement and assess the effectiveness of the support measures provided or accessed by families on improving school attendance, engagement and academic success for students.
Maintain record and information management activities including accurate case records and activity records, to ensure accountability in accordance with legislation, policy and procedures.
