Support Near Me Pty Ltd is a registered NDIS provider committed to delivering high-quality, person-centred supports that empower people with disability to achieve their goals and participate meaningfully in their community.

We are currently seeking a reliable and experienced Male Disability Support Worker to provide 1:1 community access support to an NDIS participant based in Homebush West, NSW.

Position Details

Location: Homebush West, NSW

School Term Hours

  • Monday to Friday
  • 2:45 PM – 5:45 PM

School Holiday Hours

  • Monday to Friday
  • 10:00 AM – 4:00 PM

Applicants must be available for all of the above shifts, as consistent support is essential for the participant.

About the Role

You will work independently with the participant, providing high-quality, person-centred support within the community. This role involves building a positive relationship with the participant while encouraging independence, community inclusion, and achievement of individual NDIS goals.

Key Responsibilities

  • Provide one-to-one community access support.
  • Assist the participant to engage in recreational and community activities.
  • Support social participation and skill development.
  • Promote choice, independence, and community inclusion.
  • Maintain accurate progress notes and complete required documentation.
  • Follow the participant's support plans and organisational policies.
  • Communicate professionally with families and the Support Near Me team.

Essential Requirements

  • Current NDIS Worker Screening Check.
  • Current Working with Children Check (NSW).
  • Current First Aid & CPR Certificate.
  • Current Australian Driver Licence and reliable, comprehensively insured vehicle.
  • Current Police Check (if applicable).
  • NDIS Worker Orientation Module.
  • Right to work in Australia.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and confidently within the community.
  • A professional, reliable, and person-centred approach to support.
