About the Role

We are seeking an experienced NDIS Support Coordinator (Level 2 & 3) to deliver high-quality, participant-centred services in Sydney NSW. In this permanent part-time role, you will manage a caseload of complex participants, ensuring their NDIS plans are implemented effectively and in line with their individual goals. Your focus will be on empowering participants, coordinating services across a range of providers, and building capacity for greater independence.

Key Responsibilities

Service Coordination

Manage a caseload of NDIS participants requiring Level 2 and/or Level 3 Support Coordination

Conduct participant assessments, develop action plans, and respond to referrals

Ensure participant plans are implemented effectively and in line with NDIS goals

Maintain accurate and timely records and reporting

Participant Support & Advocacy

Partner with participants to understand their needs and develop tailored support plans

Advocate for participant rights, choice, and decision-making control

Support participants to build independence, capacity, and social inclusion

Compliance & Quality

Adhere to NDIS Practice Standards and organisational policies

Monitor plan budgets, service agreements, and expenditure

Identify areas for service improvement and contribute to best practice

About You

At least 18 months in NDIS Support Coordination, including Level 2 or Level 3 Support Coordination, and/or relevant experience in Social Work, Community Services, Mental Health, Human Services, Psychology, Allied Health, Youth Work, Case Management, Advocacy, Housing, Family Support, Peer Work, or related fields.

Experience supporting participants with psychosocial disabilities, trauma backgrounds, and complex support needs. Demonstrated skills in advocacy, stakeholder engagement, recovery-oriented practice, and participant empowerment. Knowledge of Psychosocial Recovery Coaching principles and trauma-informed practice. Strong ability to build rapport and work collaboratively with participants, families, allied health professionals, and service providers.

You will be someone who:

Understands, or is willing to learn, the NDIS and disability support sector.

Has a strong commitment to human rights, dignity, choice and control, inclusion, and person-centred practice.

Can build respectful relationships with participants, families, carers, providers, and other stakeholders.

Communicates clearly, listens well, and can advocate respectfully and effectively.

Is organised and able to manage case notes, reporting, administration, and competing priorities.

Can work alongside people with different communication styles, support needs, cultures, identities, and life experiences.

Is passionate about supporting people to achieve their goals, strengthen independence, and participate fully in their communities.

Having a vehicle, with an active comprehensive insurance policy and a driver's license, is preferred but not required.
