Care Coordinator – Job Description

Key responsibilities:

  • Assess clients' care needs and develop individualized care plans.
  • Coordinate care services and schedules.
  • Conduct client assessments and regular reviews.
  • Liaise with clients, families, healthcare professionals, and support workers.
  • Monitor service quality and ensure compliance with industry standards.
  • Maintain accurate client records and documentation.
