As a Support Worker, you will provide personalised support to clients in their own homes and communities, helping them maintain their independence, wellbeing and quality of life.

About the Role

As a Support Worker, you will provide person-centred support based on each client's individual needs, preferences, goals and care plan.

Your responsibilities may include:

• Providing personal care and support with dignity and respect.
• Assisting clients with showering, dressing, grooming and other daily living activities.
• Providing domestic assistance, including cleaning, laundry and household tasks.
• Supporting clients with meal preparation and light cooking.
• Assisting with medication reminders in accordance with the client's care plan and organisational policies.
• Providing companionship and emotional support.
• Supporting clients to attend appointments, shopping and social activities.
• Providing transport and community access where required.
• Assisting clients to maintain their independence and participate in their local community.
• Supporting clients with mobility and safe movement around their home.
• Following individual care plans and reporting changes in a client's condition or circumstances.
• Maintaining accurate progress notes and required documentation.
• Communicating effectively with families, Care Partners, Nurses and other members of the care team.
• Respecting each client's culture, preferences, privacy and choices.
• Following all workplace health and safety, infection control, safeguarding and compliance requirements.
• Reporting incidents, concerns and changes in client needs promptly.
• Working collaboratively with the Pearl Home Care team to deliver high-quality care.

About You

We are looking for people who are kind, dependable, respectful and genuinely passionate about helping others.

You will ideally have:

• Previous experience as an Aged Care Worker, Support Worker, Personal Care Worker, Disability Support Worker or Community Support Worker.
• Experience working with older people or people living with disability is highly desirable.
• A genuine passion for providing person-centred care.
• Excellent communication and interpersonal skills.
• A caring, patient and respectful approach.
• Ability to build positive relationships with clients and their families.
• Reliability and commitment to scheduled shifts.
• Ability to work independently and as part of a team.
• Good observation and reporting skills.
• Ability to follow care plans, policies and procedures.
• Basic computer and smartphone skills.
• A professional and positive attitude.
• Willingness to learn and undertake ongoing training.

Qualifications & Checks

The following are highly desirable:

• Certificate III or IV in Individual Support (Ageing/Disability) or equivalent.
• Previous experience in aged care or community care.
• Current First Aid & CPR.

You will be required to have, or be willing to obtain:

• Australian National Police Check
• NDIS Worker Screening Check
• Working with Children Check, where applicable
• First Aid & CPR
• Australian working rights
• Current Australian Driver Licence
• Reliable vehicle with appropriate insurance where transport is required.
