About Purple House

Our mission is to improve the lives of people with renal failure, reunite families and reduce the impact of kidney disease in our communities. We are run by Aboriginal people for Aboriginal people and work to provide culturally appropriate health, aged care and general support services in regional and remote communities, helping people to get home to country and family.

About the Role

The primary role of the Social Support Worker of WDNWPT is to act as a ‘malpa’ or helper for dialysis patients and their families. The role of the Social Support Worker is to assist the Perth team with practical support for people with complex needs. The role is based on providing wrap around care for dialysis patients and their families that supports living their best possible life. The position works in collaborative with various teams including working alongside Perth Elder Care and Clinical Services team members.

Key responsibilities

  • Provision of practical support for clients with complex needs
  • Supports holistic care of members in collaboration with the Elder Care and Clinical team
  • Encourages, supports and participates in client social activities
  • Fosters strong relationships between clients and external stakeholders
  • Empowers clients to have their voices heard in discussions regarding their health, housing, financial, family and cultural needs
  • Advocates and assists clients to negotiate access to the health service sector
  • Demonstrates a courteous and respectful manner and communicates effectively with internal and external stakeholders
  • Documents activities and records work in daily work sheets
  • Encourages the development of positive relationships with external stakeholders
  • Communicates appropriate information to the Perth team regarding patient and service issues
  • Participates in organisational planning, implementation and evaluation activities
  • Promotes a safe work environment in accordance with legislative requirements, adheres to Work Health Safety guidelines, and reports all incidents and near misses.
  • Other duties as required within trained capacity and level.

About You

  • You have a patient centered focus and eye on our goal of getting people home on Country.
  • You are known for your compassion, empathy and ability to listen.
  • You can think on your feet to negotiate and solve problems.
  • You are considerate and thoughtful in conflict management.
  • Current National Police Check – less than 6 months old
  • First Aid & CPR or ability to obtain
  • Current C Class Australian drivers licence

Highly Desirable

  • Previous experience working for an Aboriginal organisation and awareness of the issues pertaining to this region.
  • Skills and / or experience in Social Support services
  • Food Safety Certificate or ability to obtain
