Full job description

About the role

We are seeking experienced support workers to join a small, consistent night team supporting one participant in his own home in South Granville.

This is an active night shift, not a sleepover. The participant has high physical support needs and requires 1:1 assistance across the night, including scheduled repositioning, continence management and an assisted exercise routine at both ends of the shift.

The routine is well established and predictable. What we are looking for is someone who will follow it accurately every single night, and who will document what they actually did.

About the person you will support

He is an adult man living in his own unit, who uses a wheelchair and requires full physical assistance with mobility, transfers, positioning, personal care and eating. He communicates verbally and directs his own support. He is socially connected and enjoys staying in touch with family and friends by video call and social media, watching TV and using his devices in the evening.

He knows his routine and his body extremely well. Support workers who listen to his direction and follow his instructions do well in this role.

What a typical night looks like

6:00 pm to 8:00 pm

  • Take handover from the day shift and check the communication book and any messages
  • Prepare and assist with warm water and supplements as per his routine
  • Assist with the prescribed exercise and therapy program, including assisted movement in the wheelchair and on the bed, and set up of his motorised exercise bike
  • Continence support, including urinary catheter care, and cleaning and preparing the urine bottle and bag

8:00 pm to 10:30 pm

  • Prepare the evening meal and snacks to his preference and provide full feeding assistance
  • Administer prescribed night medication as per the medication chart and sign the chart
  • Support him to connect with family and friends by WhatsApp, Messenger and video call, set up headphones, TV and devices

10:30 pm to midnight

  • Oral care and night personal care routine
  • Assisted stretching, then transfer and settle into bed with correct positioning, including cushions, palm supports and a pillow between the knees

Overnight

  • Scheduled repositioning for pressure care approximately every two to three hours, and as requested
  • Ongoing continence and catheter management through the night
  • Remain awake, alert and available in the home at all times, and respond promptly when called

5:00 am to 6:00 am

  • Morning routine, including warm water, breathing exercises, hand and leg stretches, and assisted sitting up
  • Continence support, empty and clean equipment, remove rubbish, replace bin liners, charge devices, tidy the unit
  • Complete your progress note and give a clear verbal handover to the morning shift

Essential criteria/ willingness to obtain

  • NDIS Worker Screening Check clearance
  • NDIS Worker Orientation Module certificate
  • Current First Aid and CPR
  • Certificate III in Individual Support (Disability) or equivalent, or substantial equivalent experience
  • Demonstrated experience with urinary catheter support in line with the NDIS High Intensity Support Skills Descriptors, with competency evidence
  • Medication assistance and administration training
  • Current manual handling and transfer training
  • Demonstrated experience supporting a person with high physical support needs, including full feeding assistance and scheduled pressure care
  • Ability to stay awake, alert and physically active for a full 12-hour night shift
  • Confidence to respond in an emergency, including calling an ambulance, following evacuation procedures and escalating to on call
  • Full working rights in Australia
  • Availability for a consistent, ongoing night roster, including weekends

What we offer

  • Consistent, ongoing shifts with the same participant, not a scattered roster
  • A paid buddy shift and a structured handover before you work alone
  • On call support available for the duration of every shift
