About the Department / Program

The CY-HOPE team provides wellbeing/psychosocial support, peer support and brief clinical support to young people in the 0-18 age range presenting with suicide and self-harm concerns. The Wellbeing Support Worker will work as part of the CY-HOPE team under its governance and support and, as such, will have opportunities to work in a variety of community settings.

About the Team

The Infant, Child and Youth Area Mental Health & Wellbeing Service provides assessment and treatment for infants, children, adolescents, young people and their families. We work with the 0-18 age in the Southern Region. We provide a comprehensive assessment service and a range of treatment options including inpatient treatment in times of crisis, community (outpatient) treatment, group programs, family therapy, and specialist services.

The Child & Youth Hospital Outreach Post-Suicidal Engagement (CY-HOPE) Team is a core component of Victoria’s suicide prevention framework, which aimed to halve Victoria’s suicide rate by 2025. This new service gives children and young people who are intentionally self-harming, experiencing suicidal ideation, or have attempted suicide a rapid and intensive form of support that is designed to prevent rate of recurrence and ultimately reduce rates of suicide among children and young people. Importantly, this includes engagement and support of their parents/carers, families and extended support networks in assertive outreach provided.

All stakeholders (children, young people, their parents/carers, families and service providers) will work together in this Child and Youth HOPE service at the Infant, Child and Youth Area Mental Health & Wellbeing Service at Monash Health. This service has been co-designed with and for young people and carers, First Nations people, people from Culturally Diverse Communities, LGBTIQ+ people, people in Out of Home Care, and Neurodivergent people.

The co-designed service goals are: Approachable & Accessible, Positive & Empowering, Empathetic & Personalised, Responsible & Accountable, Creative & Resourceful, and Connected & Cohesive. Infant, Child and Youth Area Mental Health & Wellbeing Service provides assessment and treatment for infants, children, adolescents, young people and their families. We work with the 0-18 age in the Southern Region. We provide a comprehensive assessment service and a range of treatment options including inpatient treatment in times of crisis, community (outpatient) treatment, group programs, family therapy, and specialist services.

About the Role

Join our team as a wellbeing support worker on a part time (0.8), fixed term (30 June 2027, with potential for this to be extended), to work at Cranbourne Community Hospital based at Cranbourne East. In this role, you will be guided under the leadership of the Team Manager and working with the broader CY-HOPE team.

Monash Health is excited to announce a Wellbeing Support Worker position in the Child & Youth Hospital Outreach Post-Suicidal Engagement (CY-HOPE) Team with the Infant, Child and Youth Area Mental Health & Wellbeing Service.

The Wellbeing Support Workers are the key workers for the young people in the CY-HOPE Team. They carry a caseload of 10 young people and provide non-clinical psychosocial support to young people and their families. This may include:

supporting engaging with friends, family, school, community programs

developing independent life skills

liaising with and supporting referrals to other mental health supports and/or family support services.

A combination of clinic based and outreach appointments

They work alongside the mental health clinicians, peer workers and consultant psychiatrist in the CY-HOPE Team to provide a meaningful approach to care that assists the young person to stay safe and connect with essential services in the first 12 weeks following discharge from hospital or contact with the service after a suicide attempt or a suicidal crisis.

The 0.8 EFT position will require you to work on Tuesdays, Thursdays and Fridays, with the other day to be of your choosing.

About You

To be successful in this healthcare role, you will bring the following demonstrated skills, experience, and attributes:

Experienced in mental health support, and/or child and family welfare support.

Strong values about the central role of the lived experience workforce, wellbeing support, and co-design in mental health service development and delivery.

Strong values about inclusion and diversity. Excellent communication and interpersonal skills.

Person centred focus, emphasis on providing choice and flexibility. Expected to be enthusiastic, compassionate & able to work collaboratively, able to prioritise and meet deadlines.

Be autonomous in work organisation and time management. Be adept in problem solving and is a solution focused person.

Mandatory requirements of the role - qualifications, licences, memberships:

Provide valid proof of your current right to work in Australia.

Complete and maintain required employment screening, including a National Police Check, employee Working with Children Check (WWCC) and, where relevant, Aged Care Worker Screening and National Disability Insurance Scheme (NDIS) Worker Screening.

Provide evidence of required immunisation prior to the extension of an offer of employment at Monash Health.

Enter into an Employment Agreement before commencement (applicants new to Monash Health only).

Comply with Monash Health’s policies and procedures, including the Code of Conduct, on an ongoing basis. Such policies and procedures do not form part of any contract of employment and Monash Health may vary the policies and procedures in its full discretion from time to time.

Demonstrate an understanding of computer systems and other technologies relevant to your role, as Monash Health uses a range of technologies and systems to support service delivery.

Current drivers licence.

Preferred experience:
An employee working in the field of social and community service who is qualified from a tertiary institution after two years’ study (one year if admission age is 21 years or over) including major studies in welfare work.

Certificate IV in Mental Health/Welfare, or minimum of one year training in other relevant qualification.

Two years experience working with clients (with children, adolescents & families).
