Chosen Family fosters a community-centric ecosystem that embraces diversity, flexibility, and enrichment for all. We continuously reimagine how our services can create platforms for equality, inclusion, and belonging. Committed to a culture of acceptance, we ensure that everyone is welcomed and valued. Recognising the interconnectedness of our humanity, we strive to empower individuals to live independently. By honouring each person’s unique identity and experiences, we celebrate and uplift one another in return.

What We are looking for:

We are seeking dedicated and compassionate individuals to join our team—people who are committed to going above and beyond for our community, our clients, and each other. We are specifically looking for experienced Disability Support Workers, particularly those with a background in supporting individuals with high behaviours and mental health challenges. A current driver’s licence is essential, as driving will be a key part of the role. If you are passionate about making a meaningful impact, we would love to hear from you.

Responsibilities of the role

  • Support clients in leading an active lifestyle and engaging with their community through person-centred programs and meaningful activities.
  • Build strong, positive relationships with people from diverse backgrounds, gender expressions, personalities, and cultures.
  • Utilise positive behaviour strategies, a person-centred approach, and a trauma-informed lens to provide effective support.
  • Assist clients with personal care, hygiene, meal preparation, medication management, community access, and appointments.
  • Maintain accurate progress notes and ensure thorough record-keeping.
  • Demonstrate strong computer, written, and verbal communication skills.
  • Apply transferable skills in providing care, empathy, and support to others.
  • Respectfully assist with personal care and domestic duties, ensuring dignity and independence.

Applicants must have:

  • Working with Children Check (WWCC)
  • NDIS Worker Screening Check
  • First Aid Certificate
  • Valid Drivers License
  • Reliable Vehicle
  • LGBTQIA+ awareness

Apply now to join our team of dedicated Disability Support Workers and make a positive impact in the lives of our clients.

We strongly encourage applications from people with a disability, Aboriginal & Torres Strait Islander people, Culturally & Linguistically Diverse people, and LGBTQIA+ people. We welcome and value your lived experience and knowledge. If you are comfortable doing so, please let us know in your cover letter if this applies to you.
