CHRH is seeking a passionate and motivated Care Partner to join our growing Care at Home team. In this rewarding role, you will work closely with older people and their families to coordinate services, support goal achievement, and deliver a positive, person-centred care experience that enables clients to remain living safely and independently at home.

What you will do:

• Coordinate and monitor services for older people living at home.
• Complete assessments, care planning and reviews.
• Build strong relationships with clients, families and service providers.
• Support clients to achieve their goals and maintain independence.
• Work collaboratively to deliver high-quality care and outcomes.

Position details:

Multiple permanent, part time positions available. Hours to be negotiated with sucessful candidates.

Salary: HS5 $48.87 per hour, plus superannuation About CHRH:
