Where you'll be working
High Street Youth Health Service and the Western Area Adolescent Team provide specialised, youth-friendly healthcare services for young people aged 12 to 24 years. Our multidisciplinary teams work alongside vulnerable, marginalised and at-risk young people to improve access to healthcare, strengthen wellbeing and support positive life outcomes. What you'll be doing
As a Youth Health Worker, you will work within a multidisciplinary team to engage, support and advocate for young people aged 12 to 24 years and their significant others. You will provide responsive, evidence-based services that improve access to healthcare and connect young people with the support they need to thrive.
Key responsibilities include:
- Delivering assertive case management, brief interventions and care coordination within a primary healthcare framework
- Building trusting relationships with young people and developing individualised care plans to address their health and wellbeing needs
- Facilitating access to health, community and support services through effective care navigation and referral pathways
- Designing and delivering health education, health promotion and group programs targeted to young people
- Collaborating with internal and external stakeholders to strengthen service delivery, advocate for young people and improve health outcomes
