Uconnect Coordination is looking for an experienced Support Coordinator to step into a Team Leader role, supporting a small team while managing a reduced participant caseload. This is an excellent opportunity for someone who is passionate about leadership, mentoring, and delivering exceptional outcomes for NDIS participants.

About the Role

As Team Leader, you'll provide day-to-day guidance and support to a small team of Support Coordinators while ensuring participants receive high-quality, person-centred services. You'll coach staff, assist with complex cases, maintain quality standards, and foster a positive and collaborative team culture.

Key Responsibilities

  • Lead, mentor, and support a small team of Support Coordinators.
  • Manage a reduced caseload of NDIS participants.
  • Provide guidance on complex participant matters, plan implementation, and NDIS processes.
  • Review case notes, documentation, and reports to ensure quality and compliance.
  • Assist with onboarding, training, and professional development of team members.
  • Monitor participant budgets and support coordinators with funding utilisation.
  • Build strong relationships with participants, families, providers, allied health professionals, and the NDIA.
  • Promote best practice and continuous improvement across the team.

About You

  • Minimum 3 years' experience as an NDIS Support Coordinator.
  • Strong knowledge of the NDIS, PACE, legislation, and operational guidelines.
  • Previous leadership, mentoring, or senior coordination experience is highly regarded.
  • Excellent communication, problem-solving, and organisational skills.
  • Ability to work independently while leading and supporting a remote team.
  • High attention to detail and commitment to quality service delivery.
  • Current NDIS Worker Screening Check.
  • Current Blue Card (or ability to obtain one).
  • Current Australian Driver Licence.
  • KPI Driven
