Youth Support Worker – Transition to Work

APM continues its impactful work through the Transition to Work contract, and we’re now seeking passionate individuals to join our Caboolture, QLD team.

As a Youth Support Worker, you’ll play a pivotal role in guiding participants toward sustainable education and employment outcomes.

Key Responsibilities:

In this dynamic and purpose-driven role, you’ll:

  • Deliver high-quality case management with a focus on individual goals and barriers to employment
  • Build strong relationships with participants, employers, and community stakeholders
  • Market clients effectively to match them with suitable employment opportunities
  • Drive performance through KPI achievement, ensuring targets are met and exceeded
  • Collaborate with your team to share insights, celebrate wins, and support each other to achieve great outcomes and drive performance

To Be Considered, You’ll Need:

  • Eligibility to work in Australia
  • A current driver’s licence and a comprehensively insured vehicle is essential
  • Willingness to complete a Criminal History Check
  • Ability to pass a Working with Children Check
  • Availability to work full-time Monday to Friday (8:30am–5:00pm)
