Community Care Worker

To assist in maintaining and enhancing the physical, emotional and social wellbeing and independence of older people and people with disabilities who are assessed as needing support to remain in their own homes.

Support and assist clients with tasks of daily living as specific in their Care Plan:

• Home Care - assisting clients with domestic responsibilities such as laundry, ironing, changing bed linen, meal preparation, tidying up, vacuuming, dusting, cleaning bathroom and toilet.
• Personal Care - assisting client with showering/bathing, toileting, dressing, grooming, and assistance with mobility, prompting of medication.
• Community Access - assisting clients with shopping, paying accounts, banking, monitoring client's safety and well-being, and any other areas of concern to the Case Manager/Coordinator.
• Support Service - in-home respite for carers, transport service to medical appointments and other activities as requested, companionship, group activities.

The office may be located in Brooklyn but we have clients all over Victoria from west to north to east to south even Morwell. Shifts can be around these areas and we are open to applicants from all over Victoria.

Job Type: Casual

Salary: From $34.86 per hour

Expected hours: No less than 10 per week
