We are seeking a dedicated Disability Support Worker to join our Transitional Rehabilitation Program (TRP) team. The role will be based at our Marangaroo site with shifts offered on a Permanent Part Time basis as per below on various days.

AM shifts: starting at 0700 with different finishing times of 1400, 1430 or 1500

PM shifts : starting at 1430 with different finishing times of 1930,2030 0r 2130

Part of the DSW’s role is to support the client’s therapy program and well-being to enable the clients to maximise their independence before they are discharged into permanent accommodation.

The TRP program provides residential based community rehabilitation to clients.

The service provides 24/7 support for individuals living with acquired brain injuries and other neurological disability (eg. Huntington’s Disease, Parkinson's Disease, Multiple Sclerosis, GBS, FND) including those with mental health needs and other comorbidities with complex cognitive, behavioural, and physical needs. This can include people with tracheostomy, Prolonged Disorders of Consciousness, TBI, stroke and neurodegenerative conditions.

Patients are referred from a Perth Metro Hospital including the State Rehabilitation Services at Fiona Stanley Hospital for continued slow stream rehabilitation.

The service delivers a highly individualised, active support person-centred and goal-oriented program through a multidisciplinary team, combining clinical support, rehabilitation programs, and behavioural strategies to maximise independence and quality of life.

The program focuses on maximising the client’s independence and potential including meaningful community engagement; working closely with families and stakeholders to ensure a coordinated, holistic approach to each client’s wellbeing prior to discharge into their long-term accommodation. Clients stay in the rehabilitation program for an average of 12-15 months.

The program interfaces with the NDIS as clients transition towards their goals and return to community. Clients are aged between 18-64 years, have access to onsite nursing, disability support workers and allied health professionals.

To be suitable for this role, you must possess:

• A Certificate III in Aged Care or Disability (or progress towards), or significant experience in a related setting and a willingness to undertake training
• Strong commitment to providing safe, high-quality care
• Excellent written, verbal and interpersonal communication skills in various environments and the ability to inclusively communicate with a diverse range of people Proven customer-focused work ethic
• Strong person-centred skills and behaviours in communicating and engaging and with clients with high support needs
• Demonstrated ability to prioritise workload, work effectively under pressure, problem solve and use resources effectively, safely and efficiently
• Current Western Australian ‘C’ Class Driver’s Licence.
