About the Community-Based Assistance Program
Our Community-Based Assistance Program provides essential daily living support and supervision for people with severe and profound disabilities. These supports are fundamental to maintaining each person's safety, well-being, dignity, and quality of life.
What makes this program unique is that all supports are delivered within community rather than in a centre-based environment.
This approach enables people to receive the personal assistance they require while actively participating in everyday community life. Whether visiting local community centres, parks, and recreation facilities; participating in leisure activities; or developing social connections, our support workers play a vital role in helping people build meaningful and authentic community relationships.
The Role
We are seeking experienced, motivated, and compassionate Disability Support Professionals to join our Community-Based Assistance team.
As a Support Worker, you will:
- Provide high-quality personal care and daily living support to people with complex support needs, including those with severe and profound disabilities.
- Support individuals who present with complex behaviours, using positive behaviour support strategies and person-centred approaches.
- Deliver both one-to-one and small group supports tailored to each person's individual goals, preferences, and support requirements.
- Implement Individual Support Plans by understanding each person's interests, strengths, goals, and aspirations.
- Promote independence, choice, and control while ensuring the safety and wellbeing of the people you support.
- Build genuine community connections by supporting people to participate in everyday activities and develop meaningful relationships.
- Work independently with confidence, exercising sound judgement and problem-solving skills in dynamic community environments.
- Collaborate effectively within a multidisciplinary team, maintaining open communication and contributing to positive outcomes.
- Maintain accurate documentation and uphold professional standards in accordance with Organisational policies and NDIS Practice Standards.
About You
You are passionate about supporting people with disability to live meaningful lives and are committed to delivering high-quality, person-centred support.
You will demonstrate:
- Experience supporting people with complex support needs, including personal care and complex behaviours.
- Excellent communication and interpersonal skills.
- The ability to work independently while remaining connected to a broader team.
- Strong problem-solving skills and the ability to adapt to changing situations.
- A genuine commitment to inclusion, dignity, respect and the rights of people with disability.
Essential Requirements
Applicants must have:
- A current Australian Driver's Licence and reliable vehicle.
- A current NDIS Worker Screening Check.
- A current Working with Children Check.
- A current National Police Check.
