About the Role:
Deliver hospital-level care where it matters most — in the homes and communities of Adelaide's north. Modbury Health Service is seeking a committed Clinical Nurse to join the Hospital In The Home (HITH) program within the Northern Adelaide Local Health Network (NALHN), bringing expert clinical care directly to patients who can recover safely outside of a hospital ward.
What You'll Be Doing
As a Clinical Nurse within the HITH team, you will manage an independent patient caseload across the northern Adelaide metropolitan area, coordinating and delivering acute-level care in community and home settings. Your key responsibilities will include:
Providing evidence-based nursing care including IV therapy, wound management, and medication administration in patients' homes
Conducting comprehensive clinical assessments and escalating care in line with NALHN protocols
Collaborating with the multidisciplinary team to plan, review, and transition patient care
Supporting patients and families with education to promote safe self-management and recovery
What You'll Bring
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA)
Demonstrated experience in acute medical, surgical, or community nursing
Confidence working autonomously in unstructured and variable care environments
Sound clinical decision-making and prioritisation skills across a mixed patient caseload
Current unrestricted South Australian driver's licence
Familiarity with SA Health clinical standards and electronic medical record systems
To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
In this role you will have strong interpersonal and communication skills which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. An ability to effectively manage multiple, diverse and competing priorities and demonstrated resilience within a stressful and complex environment is essential to this role.
