The role provides structured case management oversight during the weekday evening and weekend windows, ensuring service continuity, new client onboarding, and proactive carer matching. While not a clinical role, the

- Saturdays and Sundays- 9am and 5pm (30-minute lunch break)
You're an Enrolled Nurse coming from a client service, case management or nursing background who enjoys communicating with people. You're a problem solver and like to look for ways to continuously improve processes. You are a proactive thinker and happy to give feedback in a constructive manner if you feel a task can be completed in a more agile way. You can think on your feet and people find you engaging. You are looking for a role in which the hours suit your lifestyle.
YOU WILL BRING:
- Demonstrated experience managing complex or high-hours client portfolios across Private, and/or NDIS.
- Experience in case management, care coordination or client services within aged care, community care or disability services programs.
- Ability to work proactively and independently in a weekend or evening environment with limited supervision.
- Strong situational judgement- able to identify when clinical escalation is required and to act without delay.
- Confident communication skills: clear and professional with clients, carers and on-call contacts at all times.
- Experience with care management systems; Lookout Way preferred.
- Local to the client service area- ability to attend in the field when required.
