Key Responsibilities

  • Provide person-centred support in accordance with each participant's individual needs and support plan.
  • Assist participants with daily living activities, personal care, meal preparation, household tasks, and community participation where required.
  • Support participants to develop and maintain independence and achieve their personal goals.
  • Build positive and respectful relationships with participants while maintaining professional boundaries.
  • Provide companionship and engage participants in meaningful activities.
  • Assist with transport and community access where required.
  • Follow participant support plans, medication procedures, and workplace policies.
  • Complete accurate progress notes and other required documentation after each shift.
  • Identify and report any incidents, changes in participant wellbeing, or concerns to the appropriate team members.
  • Maintain a safe, clean, and supportive environment for participants.

About You

Requirements

  • Certificate III in Individual Support, Disability, or an equivalent qualification
  • NDIS Worker Screening Check.
  • Current First Aid and CPR certificates.
  • Medication Competency Certificate.
  • Valid Driver's licence and access to a reliable vehicle.
  • Relevant experience in disability support or community services is desirable.
  • Right to work in Australia.
