Full job description
Key Responsibilities
- Provide person-centred support in accordance with each participant's individual needs and support plan.
- Assist participants with daily living activities, personal care, meal preparation, household tasks, and community participation where required.
- Support participants to develop and maintain independence and achieve their personal goals.
- Build positive and respectful relationships with participants while maintaining professional boundaries.
- Provide companionship and engage participants in meaningful activities.
- Assist with transport and community access where required.
- Follow participant support plans, medication procedures, and workplace policies.
- Complete accurate progress notes and other required documentation after each shift.
- Identify and report any incidents, changes in participant wellbeing, or concerns to the appropriate team members.
- Maintain a safe, clean, and supportive environment for participants.
About You

- Reliable, responsible, and committed to their shifts.
- Good communicators with a positive attitude.
- Able to work independently as well as part of a team.
- Comfortable supporting participants with different needs and abilities.
Requirements
- Certificate III in Individual Support, Disability, or an equivalent qualification
- NDIS Worker Screening Check.
- Current First Aid and CPR certificates.
- Medication Competency Certificate.
- Valid Driver's licence and access to a reliable vehicle.
- Relevant experience in disability support or community services is desirable.
- Right to work in Australia.
