Employment Type: Full-time, Permanent

Salary: $79,423 per annum + superannuation

Hours: 38 hours per week

About the position

We are seeking a qualified and motivated Community Worker to join our team in Melbourne.

The Community Worker will provide direct support to individuals, families and community members, assess community and client needs, facilitate access to appropriate services and resources, and work with community organisations and service providers to improve outcomes for clients.

Key responsibilities

* Assess the needs of individuals, families and community members.

* Provide information, guidance and practical support to clients.

* Refer clients to appropriate government, health, welfare, employment, housing and community services.

* Liaise with community groups, government agencies, welfare organisations and other service providers.

* Assist clients to access community resources and support services.

* Develop and maintain positive relationships with clients and community organisations.

* Assist with the planning and delivery of community programs and activities.

* Promote awareness of available community resources and services.

* Advocate for clients where appropriate and within the scope of the role.

* Maintain accurate client records, case notes and relevant documentation.

* Prepare reports and other documentation as required.

* Participate in community outreach and engagement activities.

* Maintain confidentiality and comply with relevant legislation, policies and organisational procedures.

* Work collaboratively with colleagues and external service providers.

Essential qualifications and experience

* Relevant qualification in Community Services, Community Development, Welfare, Social Services or a related discipline.

* Relevant experience in community services, welfare support or a related field.

* Demonstrated ability to work effectively with individuals, families and community groups.

* Strong communication and interpersonal skills.

* Ability to assess client needs and identify appropriate services and referral pathways.

* Ability to maintain accurate records and documentation.

* Ability to work independently and as part of a team.

* Current Australian working rights.

Desirable

* Experience working with culturally and linguistically diverse communities.

* Knowledge of Melbourne community and support services.

* Experience working with government and non-government service providers.

* Ability to communicate in a community language.
