Position Overview

We are seeking a compassionate, reliable, and dedicated Community Support Worker (ANZSCO: 411711) to join our growing team on a full-time, permanent basis. In this role, you will be responsible for assessing the needs of individuals and families, planning and implementing relevant educational, training, and support programs, and connecting clients with appropriate community resources and services. You will work closely with individuals living with disabilities, including those with complex needs, to support their participation in community life and achievement of personal goals.

The ideal candidate will have a sound understanding of person-centred care principles, demonstrated experience working in a disability, community services, or social welfare setting, and a strong commitment to promoting the independence, inclusion, and wellbeing of the people they support. Experience working with individuals living with mental health conditions is highly regarded.

Key Responsibilities

• Provide practical and emotional support to individuals and families, including referrals to available resources, welfare agencies, and emergency supplies such as food vouchers.
• Assess individuals' and families' needs by planning, developing, implementing, and evaluating relevant educational, training, and support programs.
• Interview clients and assess the nature and extent of difficulties, including immediate issues such as housing shortages, mental health crises, and other urgent welfare needs.
• Evaluate community needs and resources related to health, welfare, housing, employment, training, and other essential services.
• Liaise with community groups, welfare agencies, government bodies, and private businesses to raise awareness and advocate for groups requiring additional assistance, promoting social inclusion and connectivity.
• Support families and educate individuals, including those requiring additional care, to develop life skills that enable independent living, while informing them of available support resources.
• Support individuals and families to actively participate in their local community and engage in meaningful recreational activities that promote healthy and active living.
• Coordinate community support services, including emergency response initiatives and delivery of resources for community programs.
• Assist individuals and families with recovery planning, goal setting, and accessing mental health and other support services.
• Monitor and report on the progress of clients, maintaining accurate records and documentation in accordance with NDIS standards and organisational requirements.
• Prepare submissions for funding and resources, and reports to government bodies and other agencies.

Required Skills and Qualifications

Qualification and Employment

• A relevant AQF Associate Degree, Advanced Diploma, or Diploma in Community Services, Social Work, Disability, or a related field. OR At least three years of relevant full-time experience in a community worker or closely related community services role may be considered in lieu of formal qualifications.
• A recent minimum of one year of relevant full-time experience working similar role within the disability or aged care sector as a support worker

Required Skills and Attributes

• Demonstrated experience assessing client needs and planning, developing, and implementing educational, training, and support programs in a community services or disability support setting.
• Sound understanding of person-centred care principles, NDIS frameworks, and the rights and needs of individuals living with disabilities.
• Experience working with individuals living with mental health conditions, complex needs, or other vulnerabilities, with the ability to provide appropriate, trauma-informed support.
• Ability to liaise effectively with community groups, welfare agencies, government bodies, and service providers to coordinate support and advocate for clients.
• Strong interpersonal and communication skills with the ability to build trust and rapport with clients, families, and stakeholders from diverse backgrounds.
• Demonstrated ability to work independently and without direct supervision while maintaining professional standards and organisational requirements.
• Current First Aid and CPR certification.
• Valid NDIS Worker Screening Check and Working with Children Check (or willingness to obtain prior to commencement).
• Current Police Check.
• A valid driver's licence and access to a reliable vehicle.
