We provide an extensive range of services, from tailored home care to enriching social activities, helping individuals achieve personal independence and fulfillment.

We are seeking a full-time Community Worker to join our team in Adelaide, SA.

Your responsibilities will include:

 Interviewing clients to understand their needs and assess difficulties they face

 Preparing plans for clients advising on support programs and implementing all aspects of the

plan, including informal, mainstream, community, and funded support.

 Following up with clients and monitor their progress and offer ongoing support and

guidance to participants to ensure their goals are met

 Referring participants to appropriate service providers and support families by providing

education on available services in the community.

 Assessing needs of the wider community for access to resources such as health, welfare,

housing, employment and other facilities.

 Liaising with community agencies about issues and promoting services/resources offered

 Preparing submissions and various reports for management and other agencies, including

implementation, progress, and end-of-plan reports.

To apply for the role, you must have the following:

 Diploma level qualification OR 3 years of work experience in lieu.

 You must also have at least 1 year of work experience in the field

The salary for the role is $79,500.
